​​What should I bring to my first appointment?

Please download, print and complete your paperwork prior to arriving at your first appointment. If you are unable to do so, please arrive at least 15 minutes prior to your scheduled appointment to complete your paperwork, and please bring your medications with you to ensure accuracy of your medical record.

Patient Policies//

Appointments: Office hours are from 8:00 AM - 5:00 PM Monday through Friday, unless otherwise stated. Please be aware that the provider's schedule varies.

 

We see patients in our office by appointment and walk-ins are always welcomed!

 

If you are going to be more than 15 minutes late, we ask that you call the office to let us know when you will be arriving so we can plan accordingly. In addition, we ask that any changes or cancellations please be made at least 24 hours in advance.

Office Delays: We make every effort to remain on schedule and minimize any inconvenience to you. However, occasionally and for reasons beyond their control, every medical office will encounter delays. Many variables can influence patient flow and can result in unpredictable waiting times. If a delay occurs, our staff will inform you as soon as possible. Should you would prefer not to wait, we will gladly reschedule your appointment for a more convenient time.

 

Medication Refills: Prescription refills are considered during office hours only. If you are currently taking a medication and would like to request a refill, it is preferred that you request refills at the time of your office visit; otherwise, please call your pharmacy directly.

 

Medication Samples: At the medical professional's discretion, prescribed medication samples may be distributed to patients when available. Samples may not always be available, so if there is difficulty in obtaining your prescription we recommend you request an alternative or contact the medication manufacturer as they may have a plan to assist you.

 

Test Results: Laboratory tests can take anywhere from a few days to 2 weeks for Vital Health to receive results. The test results are routed directly to the ordering provider to be reviewed. Our providers review and take action on all urgent results within 24-48 hours of receiving them.

 

Emergency and After Hours Calls: If you believe you have a true medical emergency, PLEASE CALL 911 IMMEDIATELY and do not attempt to contact Vital Health Primary Care.

 

Referrals and Scheduling: Vital Health providers may refer you to a specialist and/or schedule tests and procedures that are done outside of our office. Depending on your insurance plan, our staff may need to obtain an authorization before scheduling your appointment. Our staff will also be made available to assist in any scheduling needs with the external provider.

 

Medical Records Requests: To request medical records we must receive a written request form (below) with your original signature in order to release or forward your records. Because we adhere to strict confidentiality regulations (HIPAA), the request must include the patient's full legal name, date of birth, , address, and phone number of the physician/facility where the information is to be sent.  Release of Information Form

 

Questions/Concerns/Complaints/Grievances: Vital Health Primary Care staff is dedicated to providing quality health care and patient satisfaction. Should you have a concern or complaint, you may contact the Clinic Supervisor. If there is still no resolution, the Practice Administrator will explain the grievance process and other options that may be available to you.
